284 Quality improvement in venous thromboembolism (VTE) risk assessment

Abstract Aims To improve the rates of venous thromboembolism (VTE) risk assessment on elective Upper GI surgery ward in a tertiary hospital. Methods A standard 95% completion VTE within 24 hours admission was set, line with our trust policy and NICE guidance. Initial data collection for one month followed by implementation measures to assessment: raising issue departmental governance meeting, allocation responsibility based junior doctors daily checking assessments prescriptions, fortnightly snap audits check progress. Data collected from electronic patient records evaluate impact these subsequent two months. Results Initially, only 75% were completed. Following interventions, compliance increased 89% subsequently 98.5%. Conclusion is mandatory requirement all admissions potential serious harm if not assessed managed appropriately.
